Northwest Clinical Registry

When you work with Northwest Clinical Registry, you support the international relief efforts of Willamette Medical Teams (WMT). NCR is a ministry partner of WMT, which is a short-term mission organization that provides primary medical care to distressed communities in Sierra Leone and Peru. NCR offers financial support that places clinical staff where they are needed most.

You will need to print and fill out all of the following forms. All documents are in PDF format. You will need Adobe Acrobat Reader to view them.

In addition to the above documents you need to print and fill out all applicable competencies:

 - CNA / CMA
 - Emergency Room
 - ICU/CCU/CVICU
 - Labor & Delivery
 - Medical Surgery RN
 - Occupational Therapist
 - Operating Room
 - Respiratory Therapist

We will also need a copy of the following:
 - Nursing License/Certifications
 - CPR Card
 - Immunizations for Hep-B & MMR
 - Drivers License
 - Social Security Card

You will need to get a drug and TB screen from Samaritan Occupational Medicine . They are located at:
   2615 Willetta St SW Suite C2
   Albany, OR 97321.

Pre-registration through Northwest Clinical Registry is required before screening. Their office hours are Mon-Fri 7:30am-5:30pm. They can be reached at (541) 812-5600.

Northwest Clinical Registry will pay for your drug screening and your TB test as long as the test results are negative. If your test results are positive you are responsible for all applicable fees.

Application packet may be faxed, mailed, or brought into our office. If you choose to fax your documents, please follow up with a call to confirm that we have received your fax.
